We’re excited to offer **FREE ongoing case management services** designed to support your child’s unique needs and help them thrive!

Our Services Include:
Personalized Support: Available over the phone or in your community (home or a location of your choice).
Embedded Clinician: Access to a dedicated clinician within Plymouth Public Schools for in-school support.

Eligibility Requirements:
Location: Child must reside in Plymouth, MA or attend school in Plymouth, MA.
Age: Child must be aged birth to eighteen years old.
Background: Child should have a history of prenatal opioid exposure.

Our Goals:
Address and reduce educational, developmental, physical, or behavioral health challenges related to prenatal opioid exposure.
Help your child build on their natural strengths and resiliency.

About Us:
To the Moon and Back is a Plymouth-based 501(c)(3) nonprofit established in 2017. We started as a support group for parents of children with neonatal abstinence syndrome and have grown into a nationally recognized organization. Our mission includes providing grants, education, and support groups, and we’re proud to have recently received a five-year grant to advance best practices in care for children with prenatal opioid exposure.
